Surveys irritate me, too. I once filled out a survey regarding my care when I was hospitalized, but the survey was so complicated I couldn't figure out what half the questions meant, and I'm a retired medical transcriptionist with a background in medical documentation. I told the nurse that if I couldn't figure out what the survey questions were asking with my background it was a cinch people without a medical background wouldn't be able to figure it out. She agreed with me and said she'd tell her supervisor, as this was a survey that was being tested in the hospital.

Incidentally, "good care" in a hospital is highly subjective. Both Skullguy and I have been patients at this hospital and have gotten excellent care, yet I've read reviews of this hospital that say, more or less, "If you want to stay alive, don't go to this hospital." "Good service" is also highly subjective, and obviously the hierarchy of a lot of companies either haven't figured it out yet, or they don't care.
